A new treatment approach for PD may therefore consist of blocking 5-HT transporters to enhance and/or prolong the antiparkinsonian effects of drugs that have the potential to increase extracellular DA in the striatum including SSRIs.

Research has shown the implication of stress early in life with depression, a common symptom in Parkinson’s Disease. This link may provide a role for antidepressents in the treatment of Parkinson’s disease. Here Dr Ernest Dalle, author of newly published research in Molecular Brain, tells us more.
Parkinsons disease is a neurodegenerative disease characterized by loss of dopaminergic neurons in part of the mid brain called the substantia nigra pars compacta for which there is no cure. Motor symptoms of PD appear at a late stage of the disease while psychiatric disorders known as non-motor symptoms frequently precede motor symptoms by many years.
Therefore, exposure to stress during early life can have long term effects on brain development and these effects include depression. More precisely, early exposure to emotional stress such as maternal separation has been shown to cause long-term neurochemical and behavioral changes later in life.
Depression is commonly associated with Parkinsons disease with an estimated prevalence of depression in 40 -50 % of PD cases. This high prevalence of depression in PD has prompted the idea that degenerated nigrostriatal system may play a key role in depression.

BOSTONStress may be a modifiable risk factor for Parkinsons disease progression, according to research presented at the 69th Annual Meeting of the American Academy of Neurology. In a study of more than 4,000 patients, a stress proxy score predicted mortality and was associated with worsening mobility. The findings suggest that stress reduction may be an effective intervention in Parkinsons disease, said Amie Hiller, MD, Assistant Professor of Neurology at the Oregon Health and Science University in Portland.

Potentially, stress reduction is something we could think about to slow Parkinsons disease progression, said Dr. Hiller. Our goal is to not only treat symptoms of Parkinsons disease, but to slow progression of the disease.
Research suggests that stressful life events may increase the risk of Parkinsons disease. In addition, animal studies indicate that stress damages dopamine cells, resulting in more severe parkinsonian symptoms. In humans, acute stress can worsen motor symptoms, including bradykinesia, freezing, and tremor.
To examine the relationship between psychological stress and Parkinsons disease progression, Dr. Hiller and colleagues analyzed data from the National Parkinsons Foundation Quality Improvement Initiative. All 4,155 participants in the study were able to walk unassisted at baseline.
Researchers also calculated patients levels of excess stress .

- Dementia with Lewy bodies is a progressive, neurodegenerative disorder in which abnormal deposits of a protein called alpha-synuclein build up in multiple areas of the brain.
- DLB first causes progressive problems with memory and fluctuations in thinking, as well as hallucinations. These symptoms are joined later in the course of the disease by parkinsonism with slowness, stiffness and other symptoms similar to PD.
- While the same abnormal protein is found in the brains of those with PD, when individuals with PD develop memory and thinking problems it tends to occur later in the course of their disease.

Antidepressants are a popular treatment for moderate to severe forms of depression in PD . Several classes of antidepressants are available and they work in a slightly different way with different side-effects. Neurotransmitters are associated with the pathogenesis of depression in PD . Antidepressants relieve the symptoms of depression by targeting these neurotransmitters . Tricyclic, monoamine oxidase inhibitors and newer selective antidepressants including serotonin and noradrenaline reuptake inhibitors are classes of antidepressants known to be effective in treating depression .
Tricyclic drugs are the older version of antidepressants . This class of antidepressant includes drugs such as desipramine, doxepin, imipramine, trimipramine . Although tricyclic antidepressant drugs are effective in treating depression, they have several side effects such as dry mouth, constipation, difficulty urinating, sedation, weight gain or sexual problems . Also, tricyclic drugs can be fatal in overdosing .
Demographics And Clinical Characteristics
We sent our survey with specific questions relating to stress and mindfulness to all 28,385 PD patients and 11,413 controls of the Fox Insight cohort. Socio-demographic data and clinical data of PD patients had been collected earlier, immediately after enrolment in the Fox Insight cohort, and included medication use and information about the diagnosis of PD. In addition, we used responses to several validated scales that had been completed earlier, including the Unified Parkinsons Disease Rating Scale part II to evaluate daily life symptom severity, the Non-Motor Symptoms Questionnaire , the Geriatric Depression Scale , Penns Parkinsons Daily Activities Questionnaire-15 assessing daily function dependent on cognition, and the Parkinsons Disease Questionnaire-8 measuring perceived quality of life. After inclusion in the cohort, patients are asked to fill out these surveys at distinct time points throughout their individual Fox Insight participation. For respondents of our survey, we included responses to these routine surveys if they had been completed during the 12 months before receiving our additional questions. Given that the PDQ-8 was assessed more frequently, we used 6 months as a cut-off for this scale. Also, we only included scores for people who completed all items of these validated scales.

Perceived Effects Of Stress In Pd Patients
Scatterplots in Fig. show that patients who perceived high levels of stress , typically ruminated more , perceived lower quality of life , had low levels of self-compassion and scored lower on dispositional mindfulness . Furthermore, these highly stressed patients perceived higher daily life disease severity . The association between perceived stress and disease duration in the PD group was very small . All correlations were significant .
