A rough guide to when it is safe to return to work or school is:
- Bacterial pink eye: After 24 hours of antibiotic treatment.
- Viral pink eye: After 2 days to about a week.
- Allergic pink eye: No need to stay home.
Treatment Of Atypical Bacterial Conjunctivitis
Trachoma: can be treated with oral azithromycin.
Chlamydia: there is ongoing debate as to which antibiotic is most effective . Topical treatment with tetracycline ointment and systemic doxycycline or azithromycin or erythromycin .
Gonococcal conjunctivitis: only one study of the treatment of gonococcal conjunctivitis among adults has been published in recent years. In that study, all 12 patients responded well to a single 1 g intramuscular injection of ceftriaxone, together with a single episode of ocular lavage with saline.
Gonococcal ophthalmia neonatorum: early treatment and referral are essential. Although the condition is rare it should be suspected in cases of purulent conjunctivitis in neonates, since failure to treat it early can severely compromise vision. WHO guidelines suggests a single IM dose of either ceftriaxone 50 mg/kg IM, kanamycin 25 mg/kg or spectinomycin 25 mg/kg IM. Topical antibiotics are not indicated, although some sources suggest the use of topical bacitracin. Regular saline lavage is also recommended.

- Infective conjunctivitis is a self-limiting illness that usually settles without treatment within 1-2 weeks. If symptoms persist for longer than two weeks they should return for review.
- Seek medical attention urgently if marked eye pain or photophobia, loss of visual acuity, or marked redness of the eye develop.
- Remove contact lenses, if worn, until all symptoms and signs of infection have completely resolved and any treatment has been completed for 24 hours.
- Lubricant eye drops may reduce eye discomfort these are available over the counter, as well as on prescription.
- Clean away infected secretions from eyelids and lashes with cotton wool soaked in water.
- Wash hands regularly, particularly after touching the eyes.
- Avoid sharing pillows and towels.
- It is not necessary to exclude a child from school or childcare if they have infective conjunctivitis, as mild infectious illnesses should not interrupt school attendance. An exception would be if there is an outbreak of infective conjunctivitis, when advice should be sought from the Health Protection Agency by the school.
- Adults who work in close contact with others, or with vulnerable patients, should avoid such contact until the discharge has settled.
Type I Herpes Simplex
Anothernot-uncommoncause of viral conjunctivitis is the herpes virus. Type I herpes simplexbetter known as above-the-waist herpeshas at one time or another infected over 90% of the population. This form of herpeswhich is not the sexually-transmitted form of the diseaseinitially manifests as a flu-like illness in childhood. The problem isin most peoplethe virus remains in the bodyexisting in a dormant state in the nervous system. Periodicallyusually during times of stressthe virus becomes activated and causes infectionusually in the form of cold sores of the lipskin rashesor eye infections.
Currentlythere are no medications to treat adenoviral infectionbut if it is caught earlyits discomfort can be minimized. There are several medications to combat herpes eye infections. Most viral conjunctivitisalthough usually benign and self-limitingtends to follow a longer course than acute bacterial conjunctivitislasting for approximately 2-4 weeks.

What If Your Eyes Seem To Be Playing Tricks
