NEW YORK – People who undergo radiation-based tests and procedures after a heart attack may have a heightened risk of developing cancer down the road, a study published Monday suggests.

Researchers found that among nearly 83,000 Canadians whod suffered a heart attack, the risk of developing cancer over five years inched up along with the patients exposure to radiation from heart procedures.

For every additional 10 millisieverts in cumulative radiation dose received, the study found, the risk of developing cancer rose by 3 percent.

The common radiation-based procedures that heart attack patients undergo — including nuclear stress tests, heart catheterization and, increasingly, CT scans of the heart — typically deliver a radiation dose of between 5 and 15 mSv.

The study, published in the Canadian Medical Association Journal, is the latest to raise concerns about peoples increasing exposure to radiation from medical imaging. Growth in individuals potential lifetime radiation exposure has been driven largely by the growing use of CT scanning, which employs X-rays to produce three-dimensional images of the body, and nuclear medicine tests — wherein a small amount of radioactive material is injected into the bloodstream, concentrates in various tissues and is then read by special cameras.

However, the researchers stress that the cancer risk detected in the new study is small.

How Is A Nuclear Stress Test Performed

What is a Stress Test?

Your nuclear stress test will be performed in a hospital or heart clinic. The stress test takes two to five hours and generally includes these steps:

  • You will undress from the waist up and wear a patient gown for modesty.

  • Your care team will attach sticky, painless patches, or electrodes, to your chest, arms and legs. The electrodes are attached to an EKG machine by wires. The machine records your hearts electrical activity during the test. If you have a hairy chest, your provider may shave small areas to apply the electrodes.

  • Your care team will also apply a blood pressure cuff to your arm.

  • Your care team will start an IV in your arm and inject a radioactive tracer, often thallium.

  • You will rest for about 10 minutes. Then, your team will take pictures of your heart. You will need to lie still on a padded table with your arms raised overhead as a large camera rotates around you to take the pictures.

  • You will then exercise by walking on a treadmill. You will begin to exercise harder as the treadmill gradually moves more quickly and the incline increases. If you aren’t able to exercise due to a medical condition or illness, your doctor may use a drug to simulate the effect of exercise on the heart. This is called a pharmacological nuclear stress test.

  • When you reach your target heart rate, your care team will inject a different radioactive tracer, often technetium sestamibi .

  • Your team will take more pictures with the nuclear scanner 15 to 30 minutes later.

    How The Test Will Feel

    During the test, some people feel:

    • Muscle cramps in the legs or feet
    • Shortness of breath

  • After the stress test, you will wait 30 to 60 minutes. We will then take more pictures of your heart.

    What Is A Nuclear Stress Test

    The effects of radiation are many, varied, and dependent on the level of exposure. Low-level exposure can lead to major digestive imbalance, blood alteration and even the destruction of many cellular structures in the body’s key organ and tissue systems over time.     A thallium stress test is a nuclear imaging study that shows how well blood flows to the heart muscle. It usually is combined with an exercise stress test on a treadmill or stationary bicycle. The test can help doctors determine the causes of chest pain, the level of exercise you can perform safely, the extent of any coronary artery damage from a heart attack, and the effectiveness of procedures done to improve circulation in coronary arteries.

    A small amount of thallium, a radioactive substance, is injected when you reach your maximum level of exercise. Then you lie down while a gamma camera takes images of the thallium as it mixes with blood in the bloodstream, the coronary arteries and heart muscle cells. If any part of the heart muscle isnt receiving a normal blood supply, there will be less than the normal amount of thallium in those cells.

    After the first pictures are made, you lie quietly for two to three hours. Then other pictures are taken to show heart activity at rest.
