So here’s the thing: Technically, stress doesn’t directly cause acne, but it absolutely plays a role in it, as well as other skin conditions. “We know that stress has a negative impact on our skin,” says Joshua Zeichner, M.D., director of cosmetic and clinical research in dermatology at Mount Sinai Hospital in New York City. “It interferes with proper skin barrier function, impairs wound healing, and causes skin conditions like eczema and rosacea to worsen.”

“In times of stress, your stress hormones rise and trigger your oil glands to produce more oil, which then triggers acne flares, Hart explains. Hormones also play a similar role, but according to Hart, the difference is timing.
If you find you mostly get breakouts around your period, you are likely experiencing hormonal acne rather than stress acne. “Hormonal changes, mainly increased androgen levels, have the same effects on oil glands, Hart says. “The main way to tell the difference is to track acne triggers, for example, after a period of stress versus more regularly with monthly menstrual cycles.”
The location of stress acne can vary. Inflammation is the primary physical cause of acne, although excess skin oil also plays a role. Stress affects your health by causing inflammation throughout your body.
When you are under stress, your body secretes stress hormones, including cortisol. Cortisol and the other stress hormones travel through your blood stream signaling your blood vessels to dilate and your blood pressure to rise. These changes prepare you to meet the stress and they are called the fight or flight response.
Unfortunately, our modern day stresses are generally long-term and cannot be dealt with through fighting or fleeing. Chronic stress means that your stress hormones will be nearly continuously released. One effect of continual stress hormones is that your immune system never shuts down. It acts as if you are under continual attack from outside invaders . This immune system overdrive is inflammation and is a typical result of chronic stress.
Your immune system helps your body fight unwanted invaders, but too much response, inflammation, does not help your body deal with stress.
